History
St. John’s Well Child and Family Center (SJWCFC) is an independent 501(c)(3) community health center. We serve patients of all ages through a network of eighteen Federally Qualified Health Centers (FQHC). Our clinics are situated in stand-alone, school-based, community college-based, mobile units, and affordable housing development sites. The central goal of SJWCFC is to address the unmet needs of low-income, uninsured, and underinsured residents of the service area by providing access to linguistically and culturally appropriate primary medical, dental, and behavioral health services, regardless of ability to pay.
While primary health care provision is the cornerstone of our work, SJWCFC goes beyond the traditional healthcare model to address the social determinants of health with job readiness, housing navigation, household hazard remediation, and health insurance and mainstream benefits screening and enrollment. In collaboration with a wide range of public and private organizations, SJWCFC develops and implements innovative projects that include participatory research, community organizing, and advocacy. SJWCFC has established and continues to build strong affiliations with health, educational, social service and community-based and government agencies to deliver a wide range of services that are responsive to community needs.

Medical Coverage
Ninety-six percent of our patients live below 200% of the federal poverty level and seventy-five percent live below 100%.
