The California Community Foundation’s Centinela Valley Medical & Community Funds support our Care Coordination Program. The Southside Coalition in collaboration with Dignity Health-California Hospital Medical Center work together to ensure that community health center patients receive timely follow-up care after being discharged from the hospital to reduce 30-day readmissions.

The California Endowment supports our general operations allowing us to continue our health care coverage/insurance advocacy, outreach and enrollment efforts.

The Rose Hills Foundation supports our Care Coordination Program. LA Care Health Plan supports our Medical Assistant Training Program. The goal of program is to engage and provide training for medical assistants working in community health centers by giving them the tools needed to carry out their professional responsibilities; continue their professional development; and encourages them to continue working in underserved communities. The training program focuses on both the clinical and nonclinical soft skills medical assistants need to be effective members of community health center care teams. LA Care Health Plan also supports our capacity building efforts to increase the number of our member clinics who provide on-site enrollment into the Cal-Fresh Program which assists low-income patients experiencing food insecurity.

Health Net supports our health care coverage/insurance outreach, education, enrollment, and retention efforts. Southside provides targeted outreach, education, enrollment support and patient navigation to uninsured adults living in South Los Angeles. Additionally, our staff provides education on the importance of appropriately using a medical home and provides retention support and navigation for patients who currently have medical coverage to ensure they remain covered. Support and navigation are provided to patients who are hospitalized and uninsured. These patients are assisted with enrolling into a health coverage program. In addition, patients who are hospitalized and have lost coverage are assisted with re-enrollment.

The Ralph M. Parsons Foundation supports our Care Coordination Program. The Southside Coalition in collaboration with Dignity Health-California Hospital Medical Center work together to ensure that community health center patients receive timely follow-up care after being discharged from the hospital to reduce 30-day readmissions.

The Ahmanson Foundation supported upgrades of our technological infrastructure. Through the foundations support, we were able to purchase a server, new computers and a high-quality printer. Funds also supported the technology needs of our Care Coordination Program health information exchange system and the development of our new website.

Blue Shield of California supports the development Mental Health First Aid Training for Community Health Centers in South Los Angeles.

The F Cancer Foundation is supporting the implementation of a Colon Cancer Screening Project with our members to increase the number patients who receive a colon cancer screening. In addition, we will also implement a community education and outreach plan to provide education and awareness about colon cancer and the importance of colon cancer screenings.

The Delta Dental Foundation supported our Oral Health Education & Outreach campaign to educate patients covered under Medi-Cal of their dental benefits covered under that program. Funds also helped increase our Oral health outreach and education activities to educate patients about the importance of receiving regular dental care.

Cedars Sinai generously provides funding to support our general operations and our quality improvement efforts. They have graciously provided funding for our Colon Cancer Screening Project and general operating support to aid us in our efforts to strengthen the healthcare safety net and increase access to care.

Sponsored the SPA 6 Primary Care and Behavioral Health Integration Summits. These forums brought together primary care, behavioral health and substance use treatment providers to discuss ways to collaborate, improve, and increase the integration of primary care and behavioral health services in Service Planning Area Six (South Los Angeles).

Sponsored the 2019 Walk4Health and Back2School Community Health and Resource Fair. The event includes: A 2 mile Walk4Health; Free Health + Dental Screenings, Educational Resources and More; Backpack give-away with school supplies; Free food and beverages including fresh fruit and healthy snacks; a mobile food pantry; Free professional haircuts; Face-painting, petting zoo, and children’s play area.
